Initiation of Take-Home Medication Dosing and Methadone Maintenance Treatment Effectiveness
https://jamanetwork.com/journals/jamainternalmedicine/fullarticle/2853615

Methadone maintenance treatment (MMT) arguably remains the most effective medication treatment for opioid use disorder (OUD), but concerns about medication diversion and safety have historically limited MMT access. Limited data on take-home medication dosing during MMT has contributed to striking national differences in the approach to take-home dosing. The COVID-19 pandemic and calls among people who use drugs for better MMT services recently drove many nations to expand take-home dosing, creating opportunities for comparative evaluation of take-home dosing and MMT effectiveness.
Chronic Obstructive Pulmonary Disease–Related Years of Life Lost as a Prognostic Signal
https://jamanetwork.com/journals/jamainternalmedicine/fullarticle/2853613

To the Editor Bhatt and colleagues have shown that chronic obstructive pulmonary disease (COPD) is associated with a shorter lifespan, even among people who never smoked. This is important for patients and clinicians. The next message is more subtle. The reported years of life lost should be read as a warning sign from baseline airflow obstruction, not as a direct measure of how many lost years COPD itself caused or how many years treatment can restore.

Neuroprotective Therapy for Acute Ischemic Stroke
https://jamanetwork.com/journals/jama/fullarticle/2854044

A remarkable era in acute ischemic stroke care is now coming to a close—the period of the “reperfusion revolution.” Over the past few decades, pharmacological and mechanical flow restoration therapies became steadily more effective. By restoring blood flow and terminating the ischemic period, reperfusion salvages those cerebral tissues not yet irreversibly injured. However, the reperfusion strategy in acute stroke care is now reaching its limits. Current endovascular thrombectomy approaches reopen over 85% of occluded arteries, leaving little scope for further advance. And successful reperfusion treatment alone clearly only partially solves the therapeutic challenge. Among patients with ischemic stroke due to large-vessel occlusion who receive endovascular thrombectomy, more than two-thirds nevertheless have a disabled final outcome. Restoring blood flow yields incomplete recoveries because some irreversible injury accrues before recanalization can be achieved and additional injury is generated by reperfusion itself. To fully eradicate stroke-related disability, complementary therapies are needed, including: bridging neuroprotection agents, which allow brain cells to tolerate ischemia until reperfusion is attained; reperfusion neuroprotection agents, which avert reperfusion injury; and neuroreparative agents, which promote postinjury reorganization of neurons and synapses.
