المحاضرة السادسة

#زراعة_الأسنان #implant

في هذه المحاضرة سنتعرف على :-
1- جلسة كشف الزرعات "والتي تكون بعد عدة أشهر من الزراعة "
2- طرق أخذ الطبعة/القياس impression ( بطريقتين open أو closed ) مرفق بفيديو

بعد مرور الفترة الكافية لحدوث osseointegration يأتي دور كشف الزرعات و أخذ القياسات .
ملاحظة : وضحنا كيفية معرفة الفترة اللازمة سابقا (خلال المحاضرة الخامسة "تعتمد بشكل أساسي على torque و طبيعة العظم الذي تم شرحه أيضا في المحاضرة 2 )
بالوضع الطبيعي ننتظر في الفك العلوي 3-4 أشهر
بينما الفك السفلي 2-3 أشهر

خطوات كشف الزرعة :
✅1- تأكد من التزام المريض بالتعليمات و تأكد من الهيستوري قبل البداية
✅2- صور أشعة و تأكد من أن الزرعات ملتحمات في العظم وجميع ما يحيطها أبيض مثل كثافة العظم و لا يوجد أي مناطق داكنة حول الزرعة على الاشعة "كما في صورة 1"
✅3- نخدر موضعيا اللثة "infiltration"
✅4- نعمل خط الجرح فوق الزرعة بخط مستقيم و يفضل أن يكون أقرب للمنطقة اللسانية crestal incision slight lingually
انتبه للصور 2 : وهذا مهم جدا لتفادي حدوث انحسار للثة "Recession "بالناحية الظاهرة التي قد تؤثر على الجمال !
✅5- نرفع اللثة بكامل سماكتها لكشف العظم و الزرعة full-thickness flap
✅6- بعض الأحيان يكون قد بنى عظم فوق الزرعة :نحتاج لازالته بحذر باستخدم Round bur مع التبريد الجيد . لأن ارتفاع حرارة العظم عن 47 س تؤدي لدمار في خلايا العظم .
✅7- نضع gingival former و نشده بقوة torque تساوي 10 ن.سم " كما في الصورة 3"
يجب اختيار gingival former مناسبة لأنه ستشكل اللثة :
مثلا اذا كان السن المراد تعويض عريض : يجب أن تكون عريضة بنفس الشكل و ذلك ليظر السن كأنه طبيعي emergence profile
لاحظ صورة 4 :بعد ازالة gingival former
✅8- بعد وضع gingival former سنقوم بخياطة المكان و يجب ان تكون gingival former أعلى من مستوى اللثة ب1 ملم
✅9- اعطاء التعليمات للمريض و انهاء الجلسة
✅10- بعد مرور اسبوع يكون موعد فك الغرز Remove the sutures

بعد حدوث شفاء تام للثة يأي دور أخد القياس impression
هناك طرق عديدة جدا سنوضح أبسطها و أفضلها :- (شاهد الفيديو الموضوع في الرابط في أخر المنشور Link of the video 👇)

1- أخد قياس دون عمل ثقوب "تقنية الطابع المغلق" closed tray impression technique :
وهي تصلح عندما تكون الزرعات متوازية لبعضها البعض Minimal undercut
بينما لو أنها غير متوازية سنلجأ الى :
2-أخذ القياس مع عمل ثقوب ليتم فك الجزء المرتبط بالزرعة (بعد ما تتصلب المادة imp. material ) و يخرج بنفس اللحظة مع القياس
