{ لِكَيْلَا تَأْسَوْا عَلَىٰ مَا فَاتَكُمْ وَلَا تَفْرَحُوا بِمَا آتَاكُمْ }

### The Cancer That Wasn't There

Dr. Mohamed Osama

---

Painless bilateral submandibular swelling.

Bilateral lacrimal gland enlargement.

🔴 Bilateral salivary gland enlargement

🔴 Lacrimal gland enlargement

🔴 Multiple enlarged lymph nodes

Lymphoma!

Reactive lymphoid hyperplasia.

No definite evidence of malignancy.

Persistent dull lower back pain.

Creatinine: 1.9 mg/dL

CT Abdomen and Pelvis.

🔴 Retroperitoneal soft-tissue mass encasing the aorta and ureters.

🔴 Mild bilateral hydronephrosis.

- Lacrimal glands
- Salivary glands and lymph nodes
- Retroperitoneum

---

## 🛑 Stop & Think

Painless lacrimal and salivary gland enlargement

Lymphadenopathy

Retroperitoneal fibrosis

What single systemic disease could explain all these findings?

---

Lymphoma? Sjögren syndrome? Sarcoidosis? Castleman disease?

Serum IgG4

# 🔴 IgG4 = 420 mg/dL

Elevated IgG4 is a clue, not a diagnosis."

Histopathology:

🔬 Dense lymphoplasmacytic infiltrate

🔬 Storiform fibrosis

🔬 Numerous IgG4-positive plasma cells with an elevated IgG4/IgG ratio

# IgG4-Related Disease

Retroperitoneal fibrosis → Ureteric obstruction → Hydronephrosis → Renal impairment.

Glucocorticoid-based therapy

## ☀️ Lessons Learned

✅ Think IgG4-RD when lacrimal and salivary gland enlargement occurs alongside retroperitoneal fibrosis or other organ involvement.

✅ Serum IgG4 alone cannot confirm or exclude the diagnosis.

✅ Biopsy is often essential

✅ Retroperitoneal fibrosis can cause irreversible renal damage بسبب ureteric obstruction.

# 🩺 A Medical Story

# Three Tumors… One Diagnosis Nobody Expected!

### The Cancer That Wasn't There

Dr. Mohamed Osama

## 🎬 Scene 1 — “Doctor… Do I Have Cancer?”

A 54-year-old man walked into the Internal Medicine clinic carrying a bag full of scans and laboratory reports.

As soon as he sat down, he said:

> “Doctor… I've been going from one specialist to another for almost a year. Every time I have a new scan, they find another mass somewhere in my body!”
>
> “Has the cancer spread everywhere?”

It all started with painless swelling beneath his jaw.

Painless bilateral submandibular swelling.

A few months later, his eyelids became swollen and heavy.

Bilateral lacrimal gland enlargement.

A CT scan revealed:

🔴 Bilateral salivary gland enlargement

🔴 Lacrimal gland enlargement

🔴 Multiple enlarged lymph nodes

The first diagnosis everyone feared was:

Lymphoma!

A lymph node biopsy was performed.

But the result showed:

Reactive lymphoid hyperplasia.

No definite evidence of malignancy.

The patient felt relieved…

But the mystery was far from over.

## 🎬 Scene 2 — The Third Tumor!

A few weeks later, he developed persistent lower back pain.

Persistent dull lower back pain.

His blood tests revealed:

Creatinine: 1.9 mg/dL

Previously, his renal function had been normal.

A CT abdomen and pelvis was requested.

And then came another surprise!

🔴 Retroperitoneal soft-tissue mass encasing the aorta and ureters.

🔴 Mild bilateral hydronephrosis.

Now, three different regions appeared to be involved:

- Lacrimal glands
- Salivary glands and lymph nodes
- Retroperitoneum

One doctor said:

> “This must be lymphoma. Perhaps the first biopsy simply missed the disease.”

But another doctor asked:

> “What if these aren't separate tumors?”
>
> “What if one disease explains everything?”

## 🛑 Stop & Think

Painless lacrimal and salivary gland enlargement

\+

Lymphadenopathy

\+

Retroperitoneal fibrosis

What single systemic disease could explain all these findings?

## 🎬 Scene 3 — The Blood Test That Changed Everything

The team reconsidered the differential diagnosis.

Lymphoma? Sjögren syndrome? Sarcoidosis? Castleman disease?

Then one doctor said:

> “There's an immune-mediated disease that can mimic cancer and cause fibrosis around the aorta.”

The team requested:

Serum IgG4

The result came back:

# 🔴 IgG4 = 420 mg/dL

The resident exclaimed:

> “That's it! We've confirmed the diagnosis!”

But the doctor immediately replied:

> “Not yet… Elevated IgG4 is a clue, not a diagnosis.”

Serum IgG4 may also be elevated in other conditions, and some patients with the disease have normal levels.

The team arranged a core biopsy of the submandibular gland.

The histopathology report revealed:

🔬 Dense lymphoplasmacytic infiltrate

🔬 Storiform fibrosis

🔬 Numerous IgG4-positive plasma cells with an elevated IgG4/IgG ratio

After correlating the histology with the clinical findings and excluding malignancy and other important mimics…

The mystery was finally solved!

# IgG4-Related Disease

An immune-mediated inflammatory disease that can affect multiple organs, causing swelling, fibrosis, and tumor-like masses.

Three different regions… one disease behind them all!

## 🎬 Scene 4 — But Cancer Wasn't the Only Danger!

The resident said:

> “Thank goodness it's not cancer. So it's not that serious after all.”

The doctor replied:

> “Not serious?! The fibrosis around his ureters is already affecting his kidneys!”

Retroperitoneal fibrosis → Ureteric obstruction → Hydronephrosis → Renal impairment.

The patient underwent urgent urological assessment to determine whether urinary tract decompression was necessary.

Once the diagnosis was established, appropriate treatment was initiated:

Glucocorticoid-based therapy

The team also assessed the need for Rituximab in selected circumstances, with a long-term plan to monitor for relapse.

Over the following weeks…

His eyelid swelling gradually improved.

His salivary glands became smaller.

And for the first time in months…

He smiled again.
However, monitoring his renal function and retroperitoneal fibrosis remained essential, because some established organ damage may not be reversible.

## ☀️ Lessons Learned

✅ Think IgG4-RD when lacrimal and salivary gland enlargement occurs alongside retroperitoneal fibrosis or other organ involvement.

✅ Serum IgG4 alone cannot confirm or exclude the diagnosis.

✅ Biopsy is often essential. Histopathological findings must be interpreted alongside the clinical picture, while excluding lymphoma and other mimics.

✅ Retroperitoneal fibrosis can cause irreversible renal damage through ureteric obstruction.

## 💡 Don't Forget Also

1\. IgG4-RD can mimic malignancy.\
Particularly lymphoma and pancreatic cancer.

2\. Lymph node biopsy may be misleading.\
It may not show the characteristic fibrosis associated with the disease.

3\. Steroid response doesn't prove the diagnosis.\
Malignancy and other mimicking conditions must be appropriately excluded before relying on treatment response.

## 🎯 Final Message

In medicine…

Not every mass is cancer.

And not every abnormality in a different organ represents a different disease.

Sometimes, one disease wears many disguises.

One in the eyes…

Another in the salivary glands…

And another around the aorta.

The difference between a doctor who sees each finding as a separate problem and one who discovers the underlying diagnosis…

Is that the second doctor asks:

> “What connects all these findings together?”
