ملخص Febrile neutropenia

⭕️ Fever is define equal or greater than 38.3 or equal or greater than 38.0 for at least 1 hours

⭕️ Use empiric therapy

1-if negative culture until above 500

2-if positive culture until 7 days after nutrophil resolves

⭕️ In persisently febril after 3-5 days added vancomycin

⭕️ In use catheter and recurrent infection or antibiotic not response for 2-3 days will removed catheter

⭕️ If use vancomycin

1-If positive culture will change to susceptible anti bacterials

2-If persistent fever after 3 days of use will discontiuation of vancomycin

3-If vancomycin resistent or allergy will change to Linezolid, Quinupristin-dalfopristin, Tigecycline, and daptomycin


⭕️ If Persistently febrile after 5-7 days

1-use Amphotercin B (cause nephrotoxicity)

2-use liposomal amphotercin B (less tocix)

⭕️ Use posaconazole or Isavuconazonium if invasve refractory fungal

⭕️ The CSF (pegfilgrastin, filgrastim, and sargramostim) used in highly risk of hypotension, fungal infection and pneumonia , can stimulate the maturation and differentition of beutrophil precursors

⭕️ Risk from
ياما ادوية alkylating agents الي هن cyclophosphamide او ifossfamide يتحولن acrolin ويأثن المثانة او radition يأذيها بشكل مباشر

⭕️ Prevention

حتى نمنعها عدنا 3 طرق
1- Mensa (2-mercaptoethane sulfonate)
2- Hyperhydration (normal saline 3 L with furosemide)
3- Bladder irrigation with catheterization


⭕️ Tretment

اذا صارت راح نعالجها ب

1- local therapy العلاجات هي Alum, prostaglandins, silver nitate and formalin

2- if refractory to local can use estrogens, vasopressin, aminocaproic acid, and hyperbaric oxygen (HBO)

HBO particularly useful for raddition and chemotherapy induced cystitis

⭕️ These agents should be continued until bleeding stops

⭕️ In very severy, surgical may be warranted
