Has anyone heard about kids being administered suicide tests in schools without parental consent?

I have heard of two incidents in one school district, with one incident being a 6th grader in middle school and the other being a 6 year old in elementary school. Parents were not informed until after the test was administered, apparently by a school counselor.

I’m wondering if there are others around the country who this has happened to, and if so, where it is happening and what happened.
