This week the NHS will undergo one of the most radical โ and scandalous - changes in its history. From 1st April (the public are the fools in this), GPs will be contractually obliged to seek remote, electronic โadvice and guidanceโ from hospital โcliniciansโ (note, not necessarily doctors), making it even harder for patients to see a hospital specialist.
Does this sound part of a plan to genuinely โfixโ the NHS, as Wes Streeting vowed he would do so effusively when taking up office - or more like a tactic to ration hospital care by overriding GPs in order to massage the waiting list figures?
It is, of course, the latter, an extra layer of bureaucracy that at best will delay patientsโ access to the specialist treatment they need, at worse sacrifice those patients on the altar of fake news about โfallingโ waiting lists.
You donโt need me to point out the patient safety risks it potentially entails. We all know that time, in medicine, can be everything. The Royal College of GPs has been crystal clear: โThe use of advice and guidance should not be mandated in any areaโฆ We have heard reports of risks of delays, with tests being required before any referral, lost messages and staff without appropriate senior clinical oversight handling requests.โ
The aim, says the Times, is to reduce the number of hospital outpatient appointments by 30 million annually. And the government, in a really quite breathtaking example of political spin, is presenting this as โgoodโ for patients - as though all those people whoโve been waiting years months or even years for the first Rheumatology, Neurology or Orthopaedics appointment they so desperately need are just, you know, malingering. (I recently spoke to a patient with a new diagnosis of multiple sclerosis whoโd been waiting over six months to see a neurologist for the first time โ simply scandalous.)
I believe this is a national health scandal from a government that apparently cares more about good spin than it does about good patient care.
If you feel the same, please โ please โ write to your MP or the Secretary of State and tell them why. Please shout about this online, in the press, anywhere you can. Donโt let this slide.
Thank you.

The Beating Heart of London
"Whilst London is known for being a financial center, what is less known is that they are the center of global insurance. Lloydโs underwrites ~40% of the worldโs marine cargo. Ship sinks, port gets bombed, canal gets blocked the bill lands in London. This is why the UK punches above its weight. Not the Royal Navy. Not diplomacy. Insurance.
Control insurance and you control trade.
85% of the worldโs trade moves by sea and nearly half of it is insured through the London market.
And London doesnโt just control the global trade that moves by sea. Lloydโs and the London market are major insurers of almost everything: skyscrapers, factories, ports, satellites - entire supply chains.
You canโt participate in public markets or raise large amounts of capital without insurance...
...The price of a bowl of rice in Amman or a phone in Berlin can be traced back to a handful of experts deciding where the worldโs ships are allowed to sail and at what cost...
Justine Isernhinke
...MI6โs headquarters sits directly across the Thames River from the @IMOHQ, the worldโs maritime regulator, and a short distance from Lloydโs itself. Itโs long been speculated that intelligence has a direct pipeline from MI6 to Lloydโs. Having the best intelligence in the world is the greatest competitive edge any insurer can possess, providing them with the ability to price risk that competitors can only guess at. Itโs like insider trading but for global shipping...
"The cancellation of war risk coverage on ships isnโt the crisis. Itโs the canary.
If this hypothesis is correct, we could be looking at a systemic repricing event across global insurance marketsโฆ. the kind of cascading uncertainty that defined 2008 and COVID.
Watch Lloydโs. Watch reinsurance spreads. What Five Eyes. Thatโs where this story, and possibly Wall Street, breaks.โ
~John Konrad
As Lloydโs of London, the gold standard for maritime insurance, started cancelling policies or blowing up war risk to multiple orders of magnitude, other insurers around the world followed, some even pressured by Chinese financial interests.
Itโs a quiet way to control who ships and who doesnโt. That collapsed commercial shipping traffic through Hormuz, choking off oil shipments from the Middle East.
Now it could be argued, conveniently, mind you, that this is to prevent deaths and destruction of ships. And that may well be the case. But there is good old greed here too. Insurance companies donโt like paying out insurance, as we all know. They would not want to pay out on a claim of a ship lost to war. They also realize that they literally have us between a strait and a hard place, so what a great time to make A LOT more money.
London has been the center of gravity for marine insurance. Lloydโs and its reinsurers control pricing, terms, and risk appetite for global shipping. That concentration is exactly what made the actuarial blockade possible. A handful of firms in one city froze global oil flows.
Then the US steps in to underwrite the risk when the private market wonโt.
This unseats Lloydโs from their dominant position. It removes the actuarial blockade and literally replaces the global marine war risk market during an active conflict. The structural implications are significant.
This move redirects billions in premium revenues to America. Ship owners get cheaper insurance."

INTERNATIONAL LAW: Nuremberg Principle VII
COMPLICITY in the commission of a crime against peace, like participating in a war of aggression, or a war crime, like helping the bombing of an inhabited civilian target like a school or stadium, or any other crime against humanity is itself a crime under international law.
This principle emphasizes that ALL individuals, politicians & journalists included, can be held accountable for their involvement in such crimes, even if they did not directly commit them.

Pressure Mounts on Europe Diesel Supply as Tankers Turn Away
Tankers that were carrying diesel toward Europe have changed course at sea, as the Iran war boosts prices and intensifies competition for supplies.
(Bloomberg) โ Tankers that were carrying diesel toward Europe have changed course at sea... Four tankers โ the Aliai, Minerva Vaso, Grand Ace6 and Elka Delphi โ recently loaded diesel-type fuel in the US and started to cross the Atlantic, according to Vortexa and ship-tracking data compiled by Bloomberg News. The Aliai was signaling Gibraltar, while the other three were showing Amsterdam.
Since then, the ships have all made sharp turns: the Grand Ace6 is now signaling Lome in the West African nation of Togo, while the other three vessels are heading southeast. Combined, theyโre carrying about 1.2 million barrels of diesel-type fuel, Vortexa data show.
โFor April, overall we will see a sharp drop in imports into Europe,โ said George Shaw, an oil analyst at Kpler, referring to diesel-type fuels."
