A 7 year-old boy injured his foot after stepping on a nail, the wound is deep and contaminated with soil. The child is not immunized against tetanus and his parents refuse immunization because of religious beliefs. What is the most appropriate action?
A. Consult child protective services and/or obtain a court order
B. Respect the patientโs religious preferences, clean and irrigate the wound
C. Immunize the child and disregard his patientโs wishes
D. Refuse to treat the patient
Answer: A, Although you can give life-saving treatment to a child without consent, in this case, Life-altering consequences will not necessarily occour immediately, there is a window of about 1-2 days after which the tetanus vaccine/immunoglobulin may become uneffective. A more appropiate approach is to try to convince the parents and if not, call child protection services.
I found an identical case scenario by the American Academy of Pediatrics breaking down the ethical dilemma and what actions to take as a physician. I highly recommend reading through it as it will help you answer similar ethics questions.
