When it comes to questions like this, you need to read the question carefully in the exam as I suspect the recalls are missing parts of the question.

Here the physician made a call out when he said “charge nurse hang another bag of blood”

The nurse made a check back when she said “hanging the bag”

The physician should then close the loop by saying “thats right” or “correct”

Did the question ask about the type of communication the physician made? In that case the answer is call out. Did it ask about the communication the nurse made in response? In that case the answer is check back. Did the physician say “thats right” in the case and they asked about the communication in general? Then the answer is closed loop communication.

D - DNR should be reviewed every 3 months for pediatric age group

There are a few details that are missing in this case. If he is an inpatient then DNR should be reviewed every 6 months not annually. Regardless, if there is any change in the patient’s condition then DNR must be reviewed irrespective of duration. Personally I would go with B. 🫡🧠
