Patient came to the ER with MI and needed PCI, when the cardiologist assessed his condition he noticed the patient was depressed with low mood, the patient refused the PCI. He demonstrated a good understanding of his problem to the doctor . Next step?
Alot of people messaged me about this question, so far Iโ€™m still convinced the answer is further assessment then take consent.

I found a similar case scenario about a patient who is showing signs of depression and refusing dialysis. The approach was to further assess the patientโ€™s competence as it is unethical to accept consent when the patient is not emotionally competent.

For the question of PCI and depression, regarding the competency they mentioned that he has good understanding of his own problem so yes heโ€™s competent, another recall came with psychologist evaluation instead of psychiatrist so yes I believe C is the right answer not evaluate his status then consent.

The issue here is not his understanding of his problem, it is his ability to make emotionally sound decisions which are not influenced by a mental illness such as depression. Depression can cause people to be suicidal and that could be the reason he is refusing PCI, not because he doesnโ€™t understand that he is having an MI. This is why further assessment is important. If depression was found to be influencing his decision then the physician can refuse to listen to the patient and continue life saving treatment.

Why not confirm first then report I donโ€™t get it?

Smlebrain: I based my answer on the Saudi ministry of health guidelines for Mers-Cov. It clearly says report suspected cases within 24 hours. ๐Ÿง ๐ŸŒ
