Glucagon
Glucagon is synthesised by ⍺-cells in the pancreatic islets, and the main stimulus for its
release is falling blood glucose levels. Falling blood fatty-acid levels also increase
glucagon secretion. It stimulates glycogen breakdown in the liver, releasing glucose, and
breakdown of adipose and muscle tissue, releasing fatty acids and amino acids,
respectively. Its metabolic actions therefore oppose those of insulin, and it is given
subcutaneously, intravenously, or intramuscularly to increase blood glucose levels in
insulin-induced hypoglycaemia. Glucagon also has an adrenaline-like effect on the heart
and is used to improve cardiac function in shock caused by β-blocker overdose. It can
cause vomiting, so it is important to protect the airway in unconscious patients treated
with glucagon

✅ يعتبر الsodium chloride خط علاجي أول.
✅ يعتبر دواء Xylometazoline خط علاجي ثاني (second choice)
2️⃣ مزيلات الاحتقان التي ممكن أن تستخدم أثناء الرضاعة
✅ ممكن استخدام oxymetazoline أو xylometazoline على شكل قطرات أنفية
✅ وأهم معلومة يجب أن تقدمها للمريض بخصوص أدوية مزيلات الاحتقان (Decongestants) أنها لاتستخدم لمدة أكثر من 5 أيام.

Advantages of the Progestogen-Only Pill (POP):

Highly effective: The typical-use failure rate is approximately 1 per 100 woman-years, similar to combined oral contraceptives (COCs). Effectiveness depends on strict adherence to timing, as the POP must be taken at the same time every day (within a 3-hour window for traditional POPs, extended to 12 hours for desogestrel-containing pills).

No interference with sexual activity: Unlike barrier methods, the POP does not require action at the time of intercourse, allowing spontaneity and no interruption during sex.

Reversible contraceptive effects: Fertility typically returns quickly after stopping the pill, often within weeks, making it suitable for women planning pregnancy in the near future.

Safe during breastfeeding: POPs do not affect milk production or composition. They are preferred over COCs in lactating women because oestrogens in COCs can reduce milk supply and potentially affect infant health.

Suitable when COCs are contraindicated: POPs are safe for women who smoke and are over 35 years old, those with a history of venous thromboembolism (VTE), hypertension, migraine with aura, or other conditions where oestrogen-containing contraceptives pose increased risk. This is because POPs lack oestrogen, which is primarily responsible for increased thrombotic risk.
