Tooth development & Eruption
• Before Birth
- at the #third_week after conception the primitive mouth has formed. Over the next few weeks the tongue, jaws and palate develop.
-During the #sixth_week formation of embryonic cells or tooth buds that eventually form into teeth commences.
-By #eight_weeks, the tooth buds of all of the primary (baby) teeth can be discerned.
-by #twenty_weeks, the tooth buds of permanent teeth start to develop.
• After Birth
Normally, the first primary teeth start to appear in the mouth around #six_months after birth.
{The primary central incisors, lateral incisors, first molars, canines and second molars appear in this order at intervals from 6-24 months.}
#NOTES
• For each tooth type, lower teeth tend to appear about two months before the uppers.
• By age two and a half years, most children have their full complement of 20 primary teeth.
• As the child grows, the jaws also grow and spaces may begin to appear between the primary teeth. This growth takes spaces for the larger permanent teeth.
• Tooth eruption times vary widely from child to child.
• From as early as age 5 years, normally the four first permanent molars, begin to appear in the mouth. These erupt at the very back of the mouth behind the last primary tooth.
• At the same time, the two primary lower central incisors begin to loosen and fall out as the permanent lower central incisors emerge in their place.
• Over the following six years or so, the remaining 18 primary teeth fall out and are replaced by permanent teeth.
• From about age 11years, the four second permanent molars appear behind the first permanent molars.
• The last teeth to appear are the third molars or wisdom teeth. Not everybody has third molars and there is considerable variation in the age at which they erupt.
• Tooth eruption;
It's penetration of teeth through the overlying gingiva and appear in the mouth.
• The teeth normally erupt when they have reached 2/3 root length.
• The eruption process occurs due to
-pressure.
-eruptive forces.
-elongation and completion of root formation.
-some studies said it have enzymatic correlation.
But the exact eruption mechanism is not clear yet.

#Lecture (2)
#Odontiasis = {Teething} = {
Teething is the process by which a baby's teeth erupt, or break through the gums. It's generally occurs between 6 to 24 months of age.
#When_do_babies_start_teething?
teething symptoms precedes the eruption of a tooth by several days. While a baby's first tooth can appear between 4 - 10 months of age, the first tooth usually erupts at approximately 6 months of age. But variations have noted a family pattern of "early," "average," or "late" teethers.
#What_are_the_signs_and_symptoms_of_teething?
• It's generally associated with gum and jaw discomfort😭 as the tooth moves beneath the surface of the gum tissue, the area may appear slightly red or swollen.
• Sometimes a fluid-filled area similar to a "blood blister" may be seen over the erupting tooth.
• The larger molars may cause more discomfort due to their larger surface area that can't "slice" through the gum tissue as an erupting incisor.
• Increased drooling. 🤤
• Restless or decreased sleeping due to gum discomfort. ⬇️😴
• Refusal of food due to soreness of the gum. 🚫🥘🤐
• Bringing hands to the mouth and try to bite everything.
• Mild rash around the mouth due to skin irritation secondary to excessive drooling.
• Rubbing the cheek or ear region as a consequence of referred pain during the eruption of the molars.
#How_long_does_teething_last?
Children will commonly have variable discomfort. Somebabies are bothered more than others during the migration through the tissues deep to the gum line. Because of their shape, molars are more likely to be associated with teething discomfort.
#When_should_you_call_the_pediatrician_about_teething_pain?
teething is so common and other symptoms such as fever, fussiness, and diarrhea are also common, both conditions may often occur at the same time. Other illnesses or disorders (such as viral infections) are much more likely to be causing fever, fussiness, and/or nasal congestion with cough and diarrhea. It is important to contact a doctor if these or other symptoms seem concerning.
🚫Do not assume that they are just from teething..

Normal blood sugar levels are as follows:
All carbohydrate eaten 🍫🍛is digested into glucose, which passes from the stomach and intestine into the blood and is the main source of energy⚡️
Glucose homeostasis depends mainly on the pancreatic hormones insulin (from the beta-cells of the islets of Langerhans) and glucagon from the alpha-cells in the islets.
Insulin facilitates glucose transport🚗 into cells and thus lowers blood sugar (glucose) levels. Glucagon stimulates the liver to convert glyco- gen into glucose, thus raising glucose levels and having the opposite effect to insulin
Diabetes mellitus (DM) is a disorder caused by an absolute or relative lack of insulin; there can be a low output of insulin from the pancreas, or the peripheral tissues may resist insulin.

#What_medications_are_safe_to_use_to_treat_teething_pain?
Some parents use topical medicines, studies haven't consistently shown their benefit. The FDA issued a warning urging avoidance of oral medications containing the topical anesthetic benzocaine (such as Orajel).
There is an association with methemoglobinemia (a complication of use of benzocaine), a rare but extremely serious complication.
Individuals Such an adverse reaction may develop upon first exposure or after several exposures to benzocaine. Any individual who has such symptoms after exposure to benzocaine should seek immediate medical attention at the closest emergency room.
• Alcohol should never be used to numb the gums.

#What_home_remedies_provide_relief_for_teething_pain?
Teething infants often feel better when gentle pressure is placed on their gums.
• gently rubbing the gums with a clean finger.
If the pain seems to be causing feeding problems,
• a different shaped nipple or use of a cup may reduce discomfort and improve feeding.
• Cold objects many help reduce inflammation as well.
• Bite on frozen washcloths and ice cubes for this purpose.
• use of cold soothing for teethers.
Medications should be used only for the few times when other home-care methods do not help.

Access Cavity prepration🦷👩⚕
The main aim of access cavity prep to provide an intrance for root canal orifice for subsequent prepration and obturation.
So we will define the access cavity as endodontic coronal preparation which enables
1- unobstructed access to the canal orifices.
2-straight line access to apical foramen.
3-complete control over instrumentation and to accommodate obturation technique.
An ideal access preparation should have :
1. An unobstructed view into
the canal.
2. A file should pass into the canal without touching any part of the access cavity.
3. No remaining caries should be present in access cavity
To do good RCT ,You should do good access cavity😇

Will be Continue.....

#PRIMARY_DENTITION_IMPORTANCE
Baby teeth are very important child’s health and development. They help him or her chew, speak and smile. They also hold space in the jaws for permanent teeth that are growing under the gums.
#Importance_of_PRIMARY_TEETH:👼
1. Eating. 🍓🍏🍌🍇
2. Smiling. 😁
3. Speaking.🤓
4. Hold space for adult teeth and guide teeth during eruption.
5. Influence child's overall health. 👶🥳
6. Have a role in facial and jaw growth.
A primary tooth usually remains in the child's mouth until a permanent tooth underneath it is ready to emerge through the gums. The roots of the baby tooth dissolve and the tooth becomes loose and falls out. The permanent tooth "comes up" a few weeks later.
#What_happen_if_the_child_loss_primary_tooth_early ??!🤔
If a child loses a tooth too early --before the permanent tooth is ready to erupt-- or if it is accidently knocked out or is removed by the dentist because of disease, #the_space_must_be_Saved.
A Space maintainer is inserted to take the place of the "baby tooth" until the permanent tooth is ready to emerge.
If teeth on either side of the open space erupt upon the empty space, there may not be room for the permanent tooth. The new permanent tooth may erupt out of its proper position and can affect positioning of other teeth.
If teeth become crowded and out of alignment with each other, then the teeth are maloccluded. Maloccluded teeth are difficult to clean, have greater chances of becoming diseased, and later might require expensive and time-consuming orthodontic treatment..

#Dental_cement
It is hard brittle material formed when powder mixed with powder .it is barrier against thermal and chemical and electrical properties
Requirements of cements:-😁👍
It should be non toxic and non irritant to the pulp.
They should be insoluble in saliva and liquids.
They should be adhesive to enamel and dentine.
Uses:- retrain restoration
-temporary filling
-to provide thermal insulation .
Dental cement Is divided into -acid base cement
-resin cement
Types :- 1-zinc polycarboxylate cement
2- zinc phosphate cement
3-zinc oxide -eugenol cement
4.glass ionomer
1-zinc polycarboxylate :-it is used as final cement retention of crown and bridge
2.zinc phosphate; - it is composed of powder(zinc oxide)
And liquid (phosphoric acid and water)
It is low viscosity ,longevity in the oral cavity, adequate strength and easily controlled but it cause irritation to the pulp and lack adhesiveness to tooth, lack of antibacterial effect as well as it is brittle so it is not usually used.
3.zinc oxide eugenol:-
They have a sedative effect on the pulp.
It is supplied as powder and liquid or tow past system
Uses:-
Temporary luting agent
-intermediate restoration
-under filling material
-root canal sealer.
🦷🦷
Glass ionomer cement.. It adheres to enamel and dentine.
Advantages🥽
-adhesive
-biocompatibility.
Fluoride release
High strength
But its disadvantages.. Long setting time, low abrasion resistance, pulp sensitivity and residual cement is difficult to remove(البقايا من الصعب ازالتها او تنظيفها )
🌹🌹
It is classified as
Type 1.for luting
Type 2.for restoration type 2.1 esthetic
type2.2. Restorative reinforced
Type 3.for liner &base
Type 4.fissure and sealent.
Type 5. Orthodontic cement
Type 6.for core build up
Uses :-
1.ant. esthetic restoration material
2.for luting
3.for eroded area
4.an orthodontic bracket adhesive

Resin cement
Every cement is composed of liquid and powder
Advantages 👍👍
Low oral solubility
-very good bond strength
-resistance to water dissolution
As well as resistance to acid dissolution
Its disadvantages 👇
Increase film thickness.
- possible leakage
-pulp sensitivity
-Excess cement is difficult to remove

الحشوات الفضية (amalgam)
Defnition 😊
It is dental filling material used to fill cavities caused by decay.
Composition 🦷
Liquid :mercury
Powder:-alloy composed of silver, tin and copper.
Uses:
Posterior teeth🦷in all surfaces of posterior teeth
Anterior teeth:-it is not usually used but occasionally in the lingual surface.
-in the cervical third (some cases)either post. Or ant.
-Core build up
Contraindications❌
1.If patient has allergy to mercury
2.when esthetics is important
3.treatment of early primary fissure caries.
Advantages 🔔
1.It is durable تبقى لفترة طويلة
2-applicable to abroad range of clinical situations.
3.least technique sensitive of all restorative material
4.short time
5.one appointment placement
6.corrosion products seal the tooth restoration interface and prevent bacterial leakage
7.cheap
8.biocompatible
9.its failure percentage is smaller than any restorative material.
Disadvantages🔕
1.some destruction of tooth tissue
2.poor esthetic qualities
3.high thermal conductivity
4.weak adhesion
5.mercury toxicity may affect CNS and kidneys
