The OK sign is a clinical test to evaluate for potential injury of the anterior interosseous nerve. It appears when the patient is unable to approximate the thumb and index finger for pinch objects or make an "OK" sign.
◦ An injury to the anterior interosseous nerve most commonly occurs with complex trauma, whereas an isolated injury is rare.
◦ A useful mnemonic for the assessment of neurological status in injuries of the upper extremity is the use of Rock, Paper, Scissors, and OK hand movements, with which the radial, median, ulnar and anterior interosseous nerves are assessed, respectively.
