๐ Heparin โ Summary
๐น Introduction
Heparin is a parenteral anticoagulant used to prevent and treat thromboembolic disorders.
๐น Classification
-Unfractionated Heparin (UFH)
-Low Molecular Weight Heparin (LMWH)
Example: Enoxaparin
๐น Mechanism of Action
-Activates Antithrombin III
Inactivates:
-Thrombin (Factor IIa)
-Factor Xa
-Prevents conversion of fibrinogen โ fibrin
๐ Works immediately (IV route)
๐น Indications
-Deep Vein Thrombosis (DVT)
-Pulmonary Embolism (PE)
-Myocardial Infarction (MI)
-Atrial Fibrillation
-During dialysis
-Prophylaxis in bedridden patients
๐น Routes
-IV (continuous infusion)
-Subcutaneous (abdomen)
โ Never IM (risk of hematoma)
๐น Dosage Monitoring
-Monitor aPTT (Activated Partial Thromboplastin Time)
Normal: 25โ35 sec
-Therapeutic range: 1.5โ2.5 times normal
๐น Side Effects
-Bleeding
-Thrombocytopenia (HIT โ Heparin Induced Thrombocytopenia)
-Bruising
-Osteoporosis (long-term use)
๐น Antidote
-Protamine sulfate
๐น Contraindications
-Active bleeding
-Severe thrombocytopenia
-Hemophilia
-Recent brain/spinal surgery
๐น Nursing Responsibilities
โ Check baseline aPTT before starting
โ Monitor platelet count
โ Assess for bleeding (gums, urine, stool)
โ Use infusion pump for IV
โ Rotate SC injection sites
โ Do not massage injection site
โ Keep antidote ready
๐น Important Exam Points
โญ Heparin is safe in pregnancy
โญ Monitor aPTT
โญ Antidote โ Protamine sulfate
