Dilekçeler CİMER üzerinden İl Sağlık Müdürlüklerine verilebilir. Dilekçeye isim yazılmalıdır. CIMER tarih ve sayısı kaydedilerek yazılı kopyası doğum yapılan hastaneye, aile sağlığı merkezine veya gerekli her yere verilebilir.
Aile sağlık merkezine gittiğinizde veya çağırıldığınızda topuk kanı ve aşılar için sizden onam belgesini imzalamanız istenir. Bu belgeyi imzalamak demek yapılacak işlemi onaylamak anlamına gelmez. İki seçenek vardır: İşleme izin verirsiniz ve kan aldırırsınız; ya da izin vermiyorum diyerek imzalarsınız. Örnek form aşağıdadır. Bunun bir örneğini veya cep telefonu ile resmini kaydedin. Onam belgelerine Avrupa Biyotıp Sözleşmesi ve yasası gereğince topuk kanı taraması için kan alınmasına onay vermiyorum diye açıklama da yazabilirsiniz. Bu belge bütün adli süreçlerde kullanabileceğiniz tek ve önemli bir dayanaktır. Bu onam belge fotokopisine CİMER dilekçe fotokopsini de ekleyin ve gerekli her yere bunları verin.
