Update management :
According to the 2026 AHA/ASA Guidelines for the Early Management of Patients With Acute Ischemic Stroke, the recommendations for Dual Antiplatelet Therapy (DAPT) using aspirin and clopidogrel—including dosages, indications, duration, and stopping protocols—are as follows:
1. Dosing Regimen (Loading vs. Daily Maintenance)
When initiating DAPT for acute stroke or high-risk TIA, a higher loading dose is administered on Day 1 to achieve rapid platelet inhibition
Clopidogrel:
Day 1 Loading Dose: 300 mg or 600 mg oral load
Daily Maintenance Dose: 75 mg once daily
Aspirin:
Day 1 Initial Dose: 160 mg to 325 mg oral load
Daily Maintenance Dose: 75 mg to 100 mg once daily
