🦷 The loss of five anterior teeth combined with severe alveolar bone deficiency presents a clinical situation in which the outcome depends on close coordination between every stage of treatment.
This case reviews the multidisciplinary rehabilitation of a patient following severe trauma, including diagnosis, treatment planning, orthodontic preparation, bone grafting, implant placement, and the 2-year outcome.
The authors place particular emphasis on implant positioning, occlusal conditions, support of the subnasal region, and long-term esthetic risks in the anterior maxilla.

When the implant is correctly positioned but the soft tissues still fail, the entire esthetic outcome is compromised ☝️
At Ricardo Kern’s hands-on course, “Soft Tissue Management Around Implants,” we will explore how to create a stable soft-tissue contour, manage cases in the esthetic zone, and improve the predictability of implant rehabilitation.
The program includes:
⏺ immediate implant placement and immediate loading protocols
⏺ creation of a biologically driven emergence profile
⏺ treatment strategies for edentulous patients and full-arch implant rehabilitation
⏺ prosthetic protocols for the esthetic zone
The hands-on session includes practical training in:
⏺ VIP-CTG
⏺ SMILE
⏺ Table-Top
⏺ CAF and T-CAF

How will a tooth move after activation — and can you predict it in advance?
In orthodontic treatment, the outcome depends not only on the selected archwire, aligners, elastics, or miniscrews.
What truly matters is the force system created by the appliance:
▪️ force direction and magnitude
▪️ the position of the center of resistance
▪️ moment
▪️ anchorage
▪️ the biological response of tissues
In Dr. Giorgio Fiorelli’s approach, biomechanics does not begin with choosing the tool. It begins with the question:
What movement do we want to achieve — and what side effects should we expect?
In this carousel, we break down a practical algorithm:
⏺ how to define the final goal
⏺ how to design the force system
⏺ how to assess whether the chosen mechanics is clinically feasible
⏺ how to predict tooth movement before activation

Predictable aligner treatment begins not with the digital setup, but with an understanding of biomechanics 🦷
Even carefully planned digital tooth movements are not always fully expressed clinically.
This may be due to insufficient anchorage control, the way forces are transmitted through aligners, the limitations of certain tooth movements, or an inappropriate choice of auxiliary mechanics.
📍 On September 3–5 in Warsaw, Giorgio Fiorelli will present the course “Aligner Biomechanics.”
The theoretical part will cover:

Today, we’re reviewing a clinical case by Pierpaolo Cortellini — treatment of tooth 11 with a deep intrabony defect in a patient with severe generalized periodontitis 🩸
The patient presented with severe inflammation, pathological tooth migration, hypermobility, and deep periodontal pockets. The first stage focused on cause-related periodontal therapy and inflammation control. The decision to proceed with regenerative treatment was made only after re-evaluation.
During surgery, a deep, wide, 10 mm one-wall intrabony defect was identified. Cortellini used a combined regenerative approach with preservation of the soft tissues and achievement of primary wound closure.
☑️ Outcome: at 1 year, there was a 7 mm gain in clinical attachment and complete resolution of tooth mobility. At 6 years, the tooth had completely and spontaneously realigned, and the result remained stable.
In the carousel, we break down the key treatment stages and clinical decisions that made it possible to preserve the tooth.
