In endodontics, a patient’s age can provide valuable clues about the root canal anatomy you may expect even before instrumentation begins.
As patients age, the continued deposition of secondary and tertiary dentin leads to a reduction in pulp chamber volume, changes in access to canal orifices, and progressively narrower, more frequently sclerotic canals.
At the same time, the clinical priorities also change:
☑️ in younger patients — preserve tooth structure and avoid unnecessary dentin removal
☑️ in middle-aged patients — account for isthmuses, oval canal areas, and complex internal anatomy
☑️ in patients over 40 — safely negotiate and shape narrow, sclerotic canals
📚 In the carousel, we review practical recommendations from the work of Michael Solomonov and co-authors for patients aged ≤20, 20–40, and ≥40 years.

Importantly, there is no universal instrumentation protocol. Instrument selection, the extent of apical enlargement, additional canal-wall preparation, and the irrigation strategy should all be based on the actual anatomy of the individual tooth, with age serving only as one clinical guide.

When the occlusion looks stable, but function breaks down 🦷
Clinicians often see this kind of case: the contacts have been adjusted, the restorations are well adapted, yet the patient keeps coming back with overload, TMJ discomfort, or a pronounced habitual chewing side.
In these situations, it is important to assess mandibular movement:
📐 how freely laterotrusion occurs
🔍 where the restriction appears
🧩 why one side becomes more active than the other
⚖️ how this relates to the occlusal plane and the masticatory pattern
In our new post, we discuss the Planas algorithm — how the functional angle helps identify asymmetry in the masticatory trajectory and better understand where to look for the cause of the imbalance.
On November 27 in Warsaw, we will host the seminar:
“Neuro-Occlusal Rehabilitation. Seminar by Carlos de Salvador Planas”

Learning Neuro-Occlusal Rehabilitation from Planas is a rare opportunity for any clinician 🦷
In such cases, the clinician needs to understand:
where function breaks down and how it relates to occlusion, mastication, and mandibular movement.
🗺 On November 27 in Warsaw, we will host the seminar
“Neuro-Occlusal Rehabilitation. Seminar by Carlos de Salvador Planas”
Program:
⏺ Introduction to Neuro-Occlusal Rehabilitation (NOR)
⏺ Physiology of mastication: Planas’ Laws
⏺ The concept of balanced occlusion in Neuro-Occlusal Rehabilitation (NOR)
⏺ Planas Appliances
⏺ Treatment planning in Neuro-Occlusal Rehabilitation

In endodontic retreatment, prognosis largely depends on how accurately the clinician identifies the cause of the initial treatment failure and assesses the amount of remaining tooth structure.
⚠️ Aggressive canal enlargement, attempting to reach the radiographic apex at all costs, or removing an obstruction can lead to further dentin loss, perforation, and reduced root strength.
At the same time, certain radiographic findings, such as minor sealer extrusion beyond the apex, do not necessarily explain persistent periapical inflammation — the source of persistent infection must be identified.
📚 In today’s slides, we review several clinical situations from Arnaldo Castellucci’s Endodontics, Volume 2: when retreatment is truly indicated, which findings should prompt a change in treatment strategy, and when a more conservative approach may help improve the tooth’s prognosis.
🇪🇺 The European Congress of Endodontics will take place on September 18–19, with Arnaldo Castellucci joining other leading experts in modern endodontics.
If you’d like to hear Castellucci in person and spend two days immersed in endodontics, join us at the Congress! Leave a + in the comments to receive more information.

🇮🇹 Arnaldo Castellucci invites you to Florence!
On September 18–19, Arnaldo Castellucci will be speaking at the European Endodontic Congress, with a lecture focusing on two technologies that directly impact the precision of endodontic treatment: next-generation apex locators and ultrasonic preparation of the endodontic access cavity.
During his lecture, Castellucci will cover:
💥 why accurate working length determination remains a critical step in endodontic treatment and the limitations of traditional methods
💥 how third-generation apex locators work and how to interpret their readings correctly
💥 why combining different methods can improve the accuracy of working length determination
💥 the role of the operating microscope and ultrasonics in modern endodontic access preparation
💥 the advantages of ultrasonic tips over traditional burs and their clinical indications
🎥 In this video, Arnaldo Castellucci personally invites colleagues to join him at the congress in Florence.
