NHTSA investigates comma.ai over 5 crashes
On September 21, 2026, NHTSA opened a formal investigation into comma.ai’s aftermarket driver-assistance devices after 5 crashes where cars running openpilot hit stopped or slow vehicles.
Two of those crashes killed three people, while four of them left 11 people injured. The federal probe covers an estimated 30,000 devices, including comma three, comma 3X, and comma four models. Some incidents may involve modified software versions known as forks.
This is the biggest regulatory problem for George Hotz’s startup since federal regulators pushed it to cancel its first product back in 2016.

⚠️ Tesla robotaxis face possible federal probe over school zone speeding
Autonomous robotaxis have been recorded speeding through active school zones. Federal safety regulators opened a review on October 9, 2026, to weigh a formal investigation into Tesla over the hazard, which involves an estimated fleet of 1,000 vehicles including Model Y cars and Cybercabs.
The review stems from a citizen petition submitted on September 23 that included video evidence and agency incident reports regarding vehicles running with the Automated Driving System engaged.
This is the third federal regulatory action targeting the company's driverless fleet in five weeks, following previous scrutiny over vehicle safety certifications and operating systems.
Regulators have 120 days from receiving the petition to decide whether to launch a formal defect investigation.
