💥 طريقة تطبيق التكنيك :

📥 أول حاجة أحط topical .. ودا متاح فى أكتر من صورة .. ( gel , spray , paste , ointment ) .. أفضلهم ال gel 👌

🔼 فايدة ال topical إنه بيخدر ال free nerve endings .. وبالتالى بيقلل الألم أثناء ال injection ..

📥 بعد كده ندى ال injection ..

🔼 بنقسمه لمرحلتين ..

💊 Buccal infiltration :

🔼 لازم نكون عارفين ال nerve اللى بيغذى كل سنة ..

💉 max. ant. teeth ➡ ant. sup. alveolar N.

💉 max. premolars ➡ middle sup. alveolar N.

💉 max. molars ➡ post. sup. alveolar N.

💊 palatal injection :

💉 max. ant. teeth ➡ Nasopalatine N.

💉 premolars & molars ➡ greater palatine N.

📥 أدى الأول ال #Buccal_infiltration .. هنستخدم short needle 👌

📥 بعد كده أحدد النقطة اللى هعمل عندها insertion ..

🔼 أعمل خطين وهميين .. واحد عمودى موازى لل long axis بتاع السنة .... والتانى أفقى على ال mucobuccal fold

🔼 نقطة تلاقى الخطين دول هى ال point of insertion 👌

📥 اعمل retraction بمراية او بصوباعى علشان أبقى شايف مظبوط

📥 أدخل بالسن بحيث يكون بزاوية 45 وأخلى ال bevel ناحية ال bone .. لحد ما أحس بال bone ..

📥 أدى ال injection بالراااحة على اد ما اقد علشان أقلل الوجع بتاع ال solution ..

📥 أحط اغلبيه ال Solution اللى فى الكربول .. حوالى 1.7ml لحد المارك اللى ف الكربول 👌

📥 أسحب السن وأبدأ بقى أدى ال #palatal_injection

📥 بحط ال needle فى ال mid way بين ال gingival margin & ال median palatine raphe .. بحيث أكون مع ال long axis بتاع منتصف السنة اللى هشتغل فيها , مع كل الأسنان ماعدا ال upper 8 هدى mesial لل long axis بتاع السنة 👌

📥 اتجاه السن بتاعى هيكون 90 على ال palatine bone .. أحط فقط 0.3ml ( بتكون painful أكتر من ال buccal infiltration )

📥 أستنى ( 2-3 دقايق ) .. بعد كده أعمل test بروب وأشوفها شغالة ولا لأ ..

📥 وبعدين أبدأ اشتغل بقى 💪✌

Importance:-

#Maxillary landmarks:🦷

1️⃣Alveolar ridge :
It considered the primary stress bearing area of the upper jaw.

2️⃣Maxillary tuberosity:
It is a bulbous extension of the residual alveolar ridge موقعها #distal to the upper 3rd molar, terminating in the hamular notch.

3️⃣Hamular notch or (pterygo maxillary notch) :
‎موقعها It is located distally to the maxillary tuberosity and the pterygoid hamulus.
‎تستخدم It is used as a land mark for the correct extension of the upper denture .
مهمة في تحديد امتداد الطقم للفك العلوي .

4️⃣Palatine fovea:

5️⃣Vibrating line:
An imaginary line across the posterior part of the palate marking the junction between the movable and static tissues of the #soft palate.
It provides posterior seal in order to obtain peripheral seal.

6️⃣Palatine torus:
If the size of the torus is too big or extended posteriorly to the posterior palatal seal, it should be #surgically removed.
If its size is small , the denture base over this area must be #relieved to prevent a fulcrum effect.
‎إذا كان حجمها كبير يؤثر على ثبات الطقم المتحرك او صناعته يتم إزالتها جراحياً،
‎اما اذا حجمها صغير لا يؤثر على ثبات او صناعة الطقم المتحرك نكتفي
‎بعمليهrelieve
‎للمنطقة بمادة الشمع او غيرها .

7️⃣Median palatine Raphe:
It is lying sagitally in the midline of the hard palate.
If prominent it may have to be #relieved to prevent the upper denture from rocking.

8️⃣Palatine rugae:
These are raised irregular transverse mucosal ridges in the anterior part of the palate

9️⃣Incisive papilla :
Help in positioning the artificial teeth.
