NTSB investigators determined that the oil leak, which began on Oct. 1, 2021, in San Pedro Bay, resulted from an anchor strike on the pipeline that occurred eight months earlier, when anchors from the containerships Beijing and MSC Danit dragged and contacted the pipeline during high winds and seas caused by a cold front. The proximity of the anchorage positions the vessels were assigned to did not give the crews sufficient time or space to heave in their dragging anchors before contacting the pipeline. Investigator footage provided glimpses from inside the Dali cargo ship that collided with the Francis Scott Bridge in Baltimore on Tuesday, March 24, causing it to collapse, reports Yahoo.

The National Transportation Safety Board (NTSB) has released an investigation report on an incident where, on April 21, 2023, a bulk carrier grounded twice in Port Dolomite, Michigan. The bulk carrier grounded while loading cargo. About 1445, after getting underway, the crew discovered flooding and damage to the hull of the vessel. No pollution or injuries were reported.
