The Dutch Safety Board has issued the following report on a mooring line fatality onboard chemical tanker RN Privodino on 28 June 2018, while entering the Noordersluis lock at Ijmuiden, en route to Amsterdam. Shortly after the port forward spring had been paid out, it unexpectedly came under severe tension. The mooring team on the foredeck was no longer able to respond adequately in good time, as a result of which the mooring line parted. The Norwegian Safety Investigation Authority (NSIA) issued a highly critical report of the cruise line and operational managers, shipyard and classification society, in regards to the incident. The NSIA report concurs with the Norwegian Maritime Authority's (NMA) earlier conclusion that inadequate oil levels in the ship's tanks were the underlying cause. However, in its contested finding, the NMA identifies possible SOLAS violations, stating that the vessel should not have departed Tromso, Norway two days prior.#NSIA #Investigation #Report

Investigator footage provided glimpses from inside the Dali cargo ship that collided with the Francis Scott Bridge in Baltimore on Tuesday, March 24, causing it to collapse, reports Yahoo.

Parting Of Mooring Ropes Lead To Near-Breakaway Incident. What happened In the morning of 20 May 2022, CMA CGM Bellini was engaged in cargo operations at the port of Brisbane, Australia, when one of its head lines suddenly parted and another one was damaged. Soon after, the mooring winch brakes of the remaining three head lines rendered and the vessel’s bow swayed off the berth by about eight metres. Отчет касается посадки на мель танкера под флагом Гибралтара «Sten Arnold» на фарватере реки Эльба 21 августа 2022 года.
Загруженный танкер «Sten Arnold» дедвейтом 16 578 тонн, построенный в 2008 году, направлялся в Гамбург, когда сел на мель на ранее неизвестной отмели в районе буя 63 на фарватере Эльбы под лоцманским сопровождением.
С осадкой в 8,55 метра «Sten Arnold» не нуждался в использовании углубленного судоходного канала.
Судно самостоятельно сошло с мели при следующем приливе и не получило повреждений.
Расследование BSU было сосредоточено на причинах появления отмелей на Эльбе и мерах, предпринятых судоходной администрацией для их обнаружения и устранения.
В своем итоговом отчете Федеральное бюро по расследованию морских происшествий (BSU) заключило, что «посадка на мель на фарватере в районе буя 63 на реке Эльба стала полной неожиданностью для экипажа «Sten Arnold».
Кроме того, BSU отмечает, что лоцман не имел возможности предотвратить инцидент, так как более подробная навигационная карта в его портативной лоцманской установке (PPU) также не содержала информации об этой отмели.#Investigation #Report #Grounding

Inlet Valve Fatigue Causes Fire Aboard Tanker.The Malta Safety Investigation Unit (MSIU) has released its final report on the fire incident aboard a chemical tanker.The safety investigation determined that the failure mechanism of the steam inlet valve to the cargo tank cleaning water heater was most likely to have been fatigue and that no sudden event precipitated the failure. When the steam valve failed, the AB was engulfed in a dense cloud of hot steam and consequently, suffered severe burn injuries. He also suffered damage to his lungs due to hot steam inhalation.#Malta #Investigation #Report

How to conduct an effective accident investigation interview. Contrary to popular belief, the aim of an accident investigation interview is not to find who is guilty and who can be blamed: it is to discover the truth of what has happened. Once that is known, measures can be put in place to stop it from happening again.
Unlike a management interview, where the manager drives things forward, in an accident investigation interview the dynamics can be totally different. Because the interviewee might have something to hide – they might have done something wrong, they might be protecting a friend – there is the potential for a hidden agenda.
That means the manager could be the one actually under scrutiny, even if they don’t realise it. The interviewee wants to know what’s going on. They will listen very carefully to what’s being asked and to what is not being asked. They will focus on how the interviewer responds to things. Fatal Injuries To Crew During Cargo Operations.On 21 July 2023, Port Gdynia was moored alongside in the port of Bata, Equatorial Guinea. Cargo loading operations were in progress when the electrotechnical officer (ETO) was found lying on deck, with a severe head injury. The crew members observed that the injured ETO did not show any signs of life and their attempts to revive him wer unsuccessful. He was eventually transferred to a hospital, where his death was confirmed.There were no witnesses to the occurrence, but the safety investigation considered it likely that the ETO may have tripped over an obstruction on deck and fallen, subsequently suffering a fatal head injury. The MSIU has issued one recommendation to the Company, aimed at ensuring that the crew members always use the safety helmets’ chin straps. #Malta #Investigation #FatalHeadInjury
