The Bahamas Maritime Authority published its report focusing on a fumigant accident onboard the Bahamas-flagged bulk carrier. On the morning of 17 May 2022, the Bahamas-flagged bulk carrier, was at anchor outside Qinzhou, China, when an ordinary seafarer collapsed in a cargo hold containing soya beans.
The alarm was raised and the chief officer who entered to help also collapsed. Both the chief officer and ordinary seafarer were recovered from the hold by a team wearing breathing apparatus. Both were transferred to a hospital ashore where the chief officer made a full recovery. The ordinary seafarer died as a result of exposure to lethal levels of phosphine gas. The Dutch Safety Board has issued the following report on a mooring line fatality onboard chemical tanker RN Privodino on 28 June 2018, while entering the Noordersluis lock at Ijmuiden, en route to Amsterdam. Shortly after the port forward spring had been paid out, it unexpectedly came under severe tension. The mooring team on the foredeck was no longer able to respond adequately in good time, as a result of which the mooring line parted. The Marine Department of the Government of Hong Kong has issued a note 33/2024 about the fatal accident that occurred on a Hong Kong-registered container vessel during mooring operations at Qingdao, China.
During the mooring operation, the third officer (3/O) was stationed on the aft deck, overseeing the final stages of securing the vessel. At this point, only one stern line remained to be tightened. Unfortunately, as the line was being tensioned, it suddenly snapped and struck the third officer in the head, leading to his immediate death. Mooring Line Accident on Algoma Integrity Leads to Fatality.On 21 October 2018, while the Algoma Integrity was mooring at the Morton Salt terminal in The Bahamas, a mooring line fouled the propeller, leading to a fatal incident. How to conduct an effective accident investigation interview. Contrary to popular belief, the aim of an accident investigation interview is not to find who is guilty and who can be blamed: it is to discover the truth of what has happened. Once that is known, measures can be put in place to stop it from happening again.
Unlike a management interview, where the manager drives things forward, in an accident investigation interview the dynamics can be totally different. Because the interviewee might have something to hide – they might have done something wrong, they might be protecting a friend – there is the potential for a hidden agenda.
That means the manager could be the one actually under scrutiny, even if they don’t realise it. The interviewee wants to know what’s going on. They will listen very carefully to what’s being asked and to what is not being asked. They will focus on how the interviewer responds to things. And they will even see if the interview can be manipulated.#Accident #Investigation #Interview

Fatal Mooring Accident on Bulk Carrier.UK MAIB has issued an investigation report on an incident where a crew member suffered fatal injuries due to being struck by a mooring line in August 2021. On the morning of August 26, 2021, the second officer of the Isle of Man-registered bulk carrier suffered fatal injuries after being struck by a recoiling mooring line. The incident occurred while the vessel was berthing at Puerto Ventanas, Chile. Marshall Islands Accident investigation report: Crew fatality on M/V Amphitrite.This report, published on January 9, 2026, by the Republic of the Marshall Islands Maritime Administrator, investigates a fatal occupational accident that occurred on March 10, 2025, aboard the bulk carrier AMPHITRITE in the Java Sea. The casualty took place while the crew was attempting a non-routine task to free the ship's port anchor, which had become stuck in the hawse pipe due to heavy clay and mud. During a third attempt to dislodge the anchor using a hydraulic jack, the anchor moved suddenly, pinning two crewmembers. Germany marine Accident investigation: Pilot overboard from the car carrier SUPREME ACE. In the early hours of 15 January 2023, the car carrier SUPREME ACE was sailing from Emden in Germany to Davisville in the United States. Since leaving the pier in Emden, the wind and sea conditions had increased more than indicated in the weather forecasts. There was a storm in the German Bight. A pilot boat transfer was becoming increasingly difficult, and ordering a helicopter at short notice no longer possible. В египетском порту Дамиетта произошел инцидент с участием двух судов, вызвавший пожар на терминале СПГ. По данным британских компаний морской безопасности Ambrey и Vanguard, наиболее вероятной причиной стал удар беспилотного летательного аппарата по плавучей установке хранения и регазификации газа (FSRU) Energos Winter, находившейся у причала.
По имеющейся информации удар пришелся по правому борту Energos Winter (138 000+ м³), возникший пожар распространился на находившийся рядом газовоз GasLog Salem. Повреждения получила инфраструктура терминала.
Зарегистрированным владельцем судна является Energos Winter Corp., оператором Wilhelmsen Ship MGMT LTD (обе компании зарегистрированы в США).
Министерство нефти Египта подтвердило факт пожара и сообщило, что работа аварийных служб позволила быстро взять ситуацию под контроль. Однако власти не подтвердили, что причиной происшествия стала атака беспилотника, призвав дождаться официальных результатов расследования. На данный момент причиной пожара по их версии является взрыв внутри машинного отделения одного из судов.
В то же время Reuters со ссылкой на Ambrey, Vanguard и источники в сфере морской безопасности сообщает, что предварительная оценка указывает именно на удар БПЛА. На момент публикации ни одна организация не взяла на себя ответственность за произошедшее.

🛟 Ночью в Мраморном море, в 18 милях южнее Силиври, в зоне разделения движения столкнулись два турецких судна. Одно из них ушло на дно примерно за 11 минут. 10 моряков пропали без вести.
Сугогруз TUĞBERK İMAMOĞLU 🇹🇷/ IMO 9003548 вышел из Искендеруна 27 августа с грузом около 4 199 тонн рулонной стали и арматуры. Курс был на Карадениз-Эрегли, через проливы в Чёрное море. Судно 1990 года постройки, около 90 метров, владелец - Zeybe Denizcilik. 1 сентября в 13:43 UTC оно прошло Дарданеллы.
Навстречу шёл танкер-химовоз ALSU🇹🇷 / IMO 9808742, около 91 метра. На корпусе танкера видимых повреждений не было обнаружено.
Около 03:15 по местному капитан ALSU фиксирует столкновение. В 03:26 срабатывает EPIRB сухогруза. В 03:41 капитан танкера докладывает в СУДС, что столкнулся с неопознанным судном.
На поверхность всплыли круги и шлюпка free fall (свободного падения). Внутри никого не оказалось. Живых на воде пока не нашли.
В районе работают 14 морских единиц, вертолёты, БПЛА ВМС. Сам ALSU тоже участвует в поиске.

🌊 Судно GRACE ROMAVICTOR выбросило на берег во время шторма в Турции.
Танкер GRACE ROMAVICTOR 🇱🇷 оказался выброшенным на мель после обрыва буксирного троса во время шторма у пляжа Чилингёз (Çilingöz Beach) на черноморском побережье Турции.
По имеющимся данным, судно буксировали в направлении Игнеады (İğneada), однако из-за непогоды оно освободилось от буксировочного троса и дрейфом было вынесено на берег.
На момент происшествия судно было без экипажа. Информация о пострадавших не поступала.
Специалистам предстоит оценить состояние судна и определить способ его безопасного снятия с мели.
