Bahamas Maritime Authority reports of a man overboard incident.On the evening of 28 March 2024, the Bahamas flagged coastal general cargo vessel was outbound from Groveport, UK. After rigging the pilot ladder, one seafarer returned to the accommodation whilst the other stayed on deck to smoke a cigarette. Around fifteen minutes later the master called the seafarer on the handheld radio to say that the pilot was ready to disembark but got no reply. Having called several times, the master went to the messroom and instigated a search on deck. At 22:50 the master informed the pilot that they had a potential man overboard at which point the pilot asked the master to raise the alarm, informed vessel traffic services and stopped engines. The pilot boat, which was about to collect a pilot from the vessel ahead, quickly started a search and they were joined by multiple air and sea assets.The search continued through the night but the seafarer’s body was found, washed ashore, next morning.#BMA #MOB #Incident

Cargo Pump Shutdown Linked to Vibration Fatigue. An incident involving an uncoordinated emergency shutdown during tanker cargo operations was reported and analyzed by IMCA (International Marine Contractors Association).
During cargo discharging at a terminal, all cargo pumps tripped simultaneously when the Emergency Shutdown (ESD) system was activated. A duty crewmember, while on a routine pump room patrol, noticed a minor leak and immediately triggered the ESD without first coordinating with the Cargo Control Room (CCR). This caused an unexpected halt to operations. The leak was later assessed as non-critical, no injuries occurred, and operations
resumed after safety confirmation.#IMCA #Tanker #Incident

M/V DALI Incident: Loose wire being the Root Cause.At a public meeting at NTSB headquarters, investigators said the loose wire in the ship’s electrical system caused a breaker to unexpectedly open - beginning a sequence of events that led to two vessel blackouts and a loss of both propulsion and steering near the 2.37-mile-long Key Bridge on March 26, 2024. Investigators found that wire-label banding prevented the wire from being fully inserted into a terminal block spring-clamp gate, causing an inadequate connection. #DALI #Incident #RootCause

Seaman Falls from Quay Access Ladder.The incident occurred while a crane was being used to unload a smaller waste container on the vessel to a larger one on the quay. To access the quay from the vessel, it was necessary to use a fixed ladder on the wall of the quay. This was in poor condition, and was missing a step halfway up. While the crew member was climbing up the ladder, the crew member was distracted by movement of the vessel’s mooring lines, just at the point of placing a foot where the missing step should have been. The crew member lost balance, fell onto the gunwale, and then into the water. There was an immediate activation of the vessel’s emergency response procedure, and the crew member was safely recovered without serious injury. #IMCA #Incident #ManFalls #QuayAccessLadder

• небезопасный способ постановки combination ladder стал обычной практикой;
• люди работали у незакрытого выреза в леерном ограждении без страховки от падения и без спасжилета;
• никто из стоявших рядом работу не остановил, хотя в компании формально действует stop work / LARP 🤡 ;
• permit to work заполняли «для галочки», а не как инструмент управления риском;
• на судне не хватало носимых радиостанций;
