Parting Of Mooring Ropes Lead To Near-Breakaway Incident. What happened In the morning of 20 May 2022, CMA CGM Bellini was engaged in cargo operations at the port of Brisbane, Australia, when one of its head lines suddenly parted and another one was damaged. Heat Exhaustion During Mooring
Causes Crew Collapse And Death. On 27 July 2023, three crew members of the Elpida GR collapsed while mooring at the port of Mesaieed, Qatar, due to heat exhaustion, resulting in the death of the bosun. The investigation found that prolonged physical labor in hot and humid conditions was a significant factor. The Marine Safety Investigation Unit (MSIU) has recommended that the
company improve onboard conditions to ensure crew comfort and has called on the flag State Administration to enhance seafarer training on heat stroke and to ensure that relevant medical guides are carried on board Maltese-registered vessels, reports MISU.#Malta #Ivestigation #Report

Enclosed Space Entry: Why Compliance Is Critical for Crew Safety. The chief officer suffered a fatal cardiac condition while working unaccompanied in an enclosed space (cofferdam) during a voyage.
Delayed medical assistance and possible undisclosed health conditions contributed to the tragic outcome.
Safety measures and compliance with medical history disclosure are emphasized in the investigation’s recommendations.
On 25 June 2021, the chief officer of a vessel traveling from Rotterdam to Gebze entered the starboard cofferdam alone to perform maintenance. Shortly after, the crew realized he was unresponsive. Emergency rescue efforts were initiated, including CPR and air evacuation, but the officer was pronounced dead at a hospital the following day, reports MSIU.#Malta #Safaty #Report #Enclosed #Space
ATSB safety recommendations seek to eliminate single point of failure. On 9 April 2022, the Liberian-flagged bulk carrier Hagen Oldendorff departed its berth with a harbour pilot on board and four tugs assisting for an outbound passage of Port Hedland’s 40 km dredged channel.During the transit, shortly after completing a turn, an electrical short circuit led to the loss of power to all of the ship’s analogue rudder angle indicators. Incorrectly believing the ship’s steering had failed, the bridge team implemented the relevant emergency response procedures for a steering failure.#ATSB #Gronding #Report
Why Safety Harness Design Matters. The chief officer (CO ) of the cargo vessel fell overboard.
The accident highlighted safety issues with equipment, including the use of single-hook safety harnesses, and inadequate risk assessment for tasks performed during voyages.
The company implemented extensive safety measures post-incident, including revising safety protocols and enhancing personal protective equipment (PPE).
Incident Overview
On 15 October 2021, the vessel , en route from Rostock, Germany, to Bekkeri, Estonia, experienced a fatal incident. The Chief Officer (CO) fell overboard while conducting maintenance on the ship’s free-fall lifeboat. Despite a swift rescue response involving a helicopter and ship maneuvers, the CO was pronounced dead due to drowning, reports MSIU.#Malta #Report #Incident

Inlet Valve Fatigue Causes Fire Aboard Tanker. The Malta Safety Investigation Unit (MSIU) has released its final report on the fire incident aboard a chemical tanker. The safety investigation determined that the failure mechanism of the steam inlet valve to the cargo tank cleaning water heater was most likely to have been fatigue and that no sudden event precipitated the failure. When the steam valve failed, the AB was engulfed in a dense cloud of hot steam and consequently, suffered severe burn injuries. He also suffered damage to his lungs due to hot steam inhalation.#Malta #Investigation #Report

Enclosed Space Rescue Turns into Triple Casualty.Transport Malta reports an incident involving enclosed space entry, leading to the loss of consciousness of three crew members aboard a motor tanker.
The Incident
On 15 May 2024, the motor tanker was engaged in bunkering operations off the Southeast coast of Malta. Shortly after the commencement of the bunkering operation, the chief engineer was found unconscious inside the starboard side slop tank. On instinct, the chief officer, followed by the master, went down the slop tank to assist the chief engineer, without the use of a breathing apparatus (BA) set. Soon after, however, both the master and the chief officer were overcome by the atmosphere inside the slop tank, fell unconscious, and had to be helped out to the main deck. All three crew members were eventually rescued out of the slop tank and transferred to a local hospital, where they recovered several days later.
