Treatment of ALL

5 elements:
Induction
CNS directed Treatment
Consolidation
Reinduction
Maintenance phase

1. Induction Therapy:
vincristine
L- asparaginase
steroid (prednisone or dexamethasone)
ال Dexamethazone هو المفضل لان يكدر يوصل لل CNS
ممکن بالـ higher risk patient نضيف دواء من مجموعة anthracycline

2. CNS prophylaxis:

A. Intrathecal:
Cytarabine
MTX
Steroid

B. Systemic chemotherapy: Dexamethasone
high-dose methotrexate

بالاضافة الى الأشعاع
craniospinal irradiation (XRT) in selected high-risk patients (T-cell ALL)

3. Consolidation
High dose of chemotherapy

4. Reinduction
A. Interim maintenance 2 months
B. Delayed intensification for 3 months after remission
نفس ادوية ال induction نعيدها من جديد

5. Maintenance phase
oral methotrexate
6- mercaptopurine.
2-3 years with low dose

Relapse ALL

Chemotherapy
Allogeneic hematopoietic stem cell transplant (allo-HSCT)

Chemotherapy:

🔸Clofarabine ➔ active in refractory acute leukemias

🔹Blinatumomab ➔ targets CD19, for Ph negative relapsed or refractory ALL

🔸Inotuzumab ozogamicin ➔ anti CD-22, used in relapsed and refractory B-ALL

🔹Tisagenlecleucel ➔ anti-CD19, used in patients with relapsed and refractory B-cell ALL

𝗔𝗥𝗢𝗡 𝗦𝗧𝗨𝗗𝗬

Treatment of AML

1. Induction
combination of cytarabine and daunorubicin

ممكن نضيف الـ gemtuzumab ozogamicin للـ induction therapy للمرضى كبار السن

2. Consolidation
use of 2-4 cycles of high-dose cytarabine

🫆 Most effective treatment for AML is Allo HSCT

في حالة الـ CNS Therapy نستخدم:
● Intrathecal Cytarabine with or without MTX
● Systemic High dose Cytarabine

🫆Relapse AML

🔸High dose of Cytarabine
May be combined with: MECCF
Mitoxantrone, Etoposide 2-Chlorodeoxyadenosine, Clofarabine, Fludarabine



🔹Patients unable to tolerate intensive chemotherapy 👆, use low dose of Cytarabine and Azacitidine

🔸Alternative therapy:
Ventoclax combined with Decitabine or Azacitidine

𝗔𝗥𝗢𝗡 𝗦𝗧𝗨𝗗𝗬

Treatment of CML

🔹first- line therapy for newly
diagnosed CP-CML is Tyrosine Kinase Inhibitors TKI

1st G: imatinib

2nd G: dasatinib, nilotinib, and bosutinib

3rd G: Ponatinib

الاخير استخدمه في حالة
CML with T315I mutation

آثارها الجانبية
Myelosuppression, gastrointestinal disturbances, and myalgias

Pleural effusion ➔
خصوصا ال imatinib و Dasatinib

🔸Hydroxyurea
هذا نستخدمه حتى نقلل اعداد neutrophil او WBC حتى نمنع حصول
Respiratory and neurologic compilations
ما عنده اي تأثير على CML

⭕ Pt suffered from CML, and taken TKI,
وما مستفاد منها يعني صايرة عنده resistant لل TKI

ف ال Curative therapy بهاي الحالة هو Allo HSCT العلاج الوحيد الي يسب cure بحالة CML

🔹Omacetaxine :
indicated for patients in CP- or AP- CML who are resistant or intolerant to two or more TKIs

كذلك في حالة وجود هذه الطفرة T315I

Ad. Effect: hyperglycemia

Treatment of ALL

5 elements:
Induction

⭕ Ibrutinib (BTK inhibitor) , ventoclax (BCL2 inhibitor) ➔ front line treatment of symptomatic CLL

🫆 ممكن نضيف
Monoclonal antibody anti CD20
للخط الاولي في علاج الـ CLL

هاي بشكل عام

نجي للتفصيل:


⭕ Cytotoxic Chemotherapy

🫆 FCR (fludarabine, cyclophosphamide, and rituximab)

مفضلة في المريض الي عنده CLL وعمره اقل من 65 سنة وعنده Igvh mutation

⭕ Monoclonal Antibody

Anti CD20

🫆 Rituximab, Obinutuzumab, Ofatumumab

ما استخدمهن كـ monotherapy, وانما انطيهن ب combination مع
Targeted and Chemo therapy

بعضها مشتق من مصادر حيوانية، فتسبب تفاعلات مناعية وتحسسية فيحتاج انه انطي قبلها :
Diphenhydramine and acetaminophen

وممكن يسون تنشيط لبعض الفيروسات مثلا Hepatitis B
ف كوقاية ننطي
Prophylactic hepatitis B antiviral therapy

واذا تفعل الفيروس نحتاج علاج والعلاج هو entecavir


⭕ Targeted therapy

BTK inhibitors:

🫆 Ibrutinib ➔ first line as monotherapy
في كل المرضى سواء الي عدهم حذف بجين P53 من کرموسوم 17 او لا

بس ننتبه انه ممكن يسبب atrial fibrillation

🫆 Acalabrutinib ➔ 2nd gen. Of BTK inhibitors, more selective than Ibrutinib

ممكن استخدمه ك خط علاجي اول لوحده او مع الـ obinutuzumab

وممكن نستخدمه في علاج الـ
Relapsed or Refractory CLL


Pi3K inhibitors

🫆Idelalisib, duvelisib

الـ idelalisib ممكن استخدمه لوحده او ب combination مع الـ rituximab


BCL2 inhibitor

🫆 Venetoclax ➔ may used as first line for pt with CLL

وممكن للمرضى الي عدهم
Relapsed Or Refractory CLL

ممكن انطي وياه Anti CD20

آثاره الجانبية :
TLS
Prolonged Neutropenia
لذلك نحتاج ننطي وياه Growth factor support
