ملخص اسباب الـ hyperthyroidism

بالنسبة لدواء الـ amiodarone فهو ممكن يسبب ‏hyperthyroidism بسبب محتواه العالي من ال iodine ممكن تمتصه ال TG
وممكن هذا الايودين يدمر ال TG ويصير overt hypothyroidism
او يصير تأثير ال Wolff–Chaikoff effect ويصير تثبيط للغدة بشكل مؤقت وممكن يستمر التثبيط ويصير subclinical hypothyroidism

Cases of Hyperthyroidism


⭕ Thionamide

1. Pt was diagnosed Hyperthyroidism given methimazole or PTU ,
symptoms improved initially, but now she has relapsed hyperthyroidism

البديل المفضل؟
preferred Use of radioactive iodine

2. patient (children or older pt) is diagnosed with Hyperthyroidism
The doctor is deciding between methimazole and PTU as initial therapy

يختار methimazole هو الخط الاول اذا المريض طفل او كبير بالعمر لان ptu يسبب hepatotoxicity

3. pregnant women has Hyperthyroidism
بعدها بالثلث الاول اول ثلاثة اشهر من الحمل او اول ١٢ اسبوع العلاج المفضل نختار PTU

ثلث ثاني او ثالث نختار methimazole

4.patient on methimazole develops severe rash and arthralgia (hypersensitivity)

نبدل لل PTU

5. patient presents with: Fever, Tachycardia, Agitation, Severe thyrotoxicosis
يعني يعاني من العاصفة Thyroid storm

نختار PTU على حساب ال methimazole

⭕ Iodide

6. A patient with Graves’ disease is scheduled for thyroidectomy in 10 days.

what drug is given before surgery?
المفضل لتحضير المريض للعملية الجراحية هو

iodide (SSKI or Lugol solution)

7. A patient receives radioactive iodine (RAI) therapy for Graves’ disease

شنو العلاج المساعد الي ممكن اضيفه بعد اعطاء ال RAI؟
الجواب iodide (SSKI or Lugol solution) او β-Blockers

اذا سأل عن علاج ممنوع انطيه بهاي الحالة؟

⭕ β-Blockers

مريض عنده وحدة من هاي الحالات وماخذ واحد من هاي العلاجات
antithyroid drugs, RAI, or iodides
لكن باقية عنده هاي الاعراض
palpitations, anxiety, tremor, and heat
intolerance

شنو العلاج المساعد adjunctive therapy الي ممكن اضيفه مع الادوية السابقة ؟
β-Blockers (Propranolol and nadolol)

وفقط بحالة وحدة استخدمه كعلاج اساسي مو مساعد primary therapy
اذا جان سبب thyrotoxicosis هو thyroiditis

فممكن يخيركم بالحالة الي استخدم بيها ال β-Blockers كعلاج اساسي

⭕ RAI

10. RAI is the agent of choice for:
Graves’ disease
toxic autonomous nodules
toxic multinodular goiters

ممنوع في Pregnancy

ف شنو لازم نسوي قبل لا ننطي RAI؟

ننطي antithyroid الي هي thionamide مثلا methimazole

او عنده asthma

واريد انطيه دواء من ادوية β-blocker؟

ما اروح لل propranolole هذا non selective ما يفيدنه

لانه selective وآمن بحالات
cardiac instability
pulmonary disease (e.g., asthma)
risk of decompensation

What is the appropriate initial dose of levothyroxine?

مريض hypothyroidism و ما عنده مشاكل بالقلب

شنو الجرعة المناسبة اله من levothyroxine ؟

الجواب : نبدي ب
levothyroxine 50 mcg daily

وبعد شهر ممكن نزيد الجرعة

2. A 72-year-old woman with hypothyroidism and a history of ischemic heart disease.

What is the safest initial dose of levothyroxine?

اما اذا المريض عنده مشاكل بالقلب فشنو الجرعة المناسبة اله؟

الجواب : نبدي ب
levothyroxine 25 mcg daily
وشهريا ممكن نزيد الجرعة بنسبة
25 mcg

⭕ الادوية الي تتداخل مع او تأثر على امتصاص الـ Levothyroxine
مهمة وممكن تجي بيها كيسات مثلا

ف من سألناه شنو جاي تاخذ وياه علاجات كال اخذ علاج ينظم ضربات القلب !!!

ف شنو الدواء الي ممكن يأثر على تحويل T4 الى T3 وهو من الادوية المستخدمة لعلاج الـ arrhythmia
